Support For Families
Seeing a loved one with Alzheimer’s or other dementia-related conditions can be daunting. To help with the transition, Prolific Care Communities helps families with the educational resources needed to familiarize themselves with the dementia process. Care center staff also refers families to support groups in their area, and invites speakers to come into the community to help families learn more about dementia and Alzheimer’s care.At Prolific Care Communities we offer two levels of memory care services, Enhance Memory Support and Advance Memory Care programs. Our neighborhoods provide a safe, secure environment and excellent care, while encouraging residents to be physically active and participate in decisions of their daily lives. Our specially trained caregivers and lifestyle coordinators focus on residents’ individual strengths celebrate their successes and find ways to help them continue to feel like themselves.

Tier II For residents with mild to moderate Alzheimer’s or other dementia-related conditions, we provide a structured, comfortable environment within our care center.
Our memory support setting helps residents adapt to their environment. We use our Holistic person-centered approach, and memory cues, to instill a sense of familiarity. Our programs and events also encourage daily connection. All of these tools have a purpose: to stimulate memory and provide residents with meaningful experiences

Tier III For residents with moderate to severe Alzheimer’s or other dementia-related conditions, we provide a structured setting allows specially trained staff to provide the best of personalized care and attention comfortable environment within our care center.
Our Advanced Memory Care setting helps residents adapt by providing comfort measures to ease the anxiety and systematic management associated with memory loss. Our Holistic person-centered approach helps to encourage a sense of familiarity. Our programs and events also encourage daily connection and tools have that allows purpose to stimulate memory and provide residents with meaningful experiences at every stage of their illness.
